With a roster of muscles ranging from the powerful glutes to the small and agile abductors, the hips control practically all your movements. Almost every endurance athlete overworks some hip muscles while underworking others, causing severe imbalances: Runners are infamous for having weak hip adductors—the muscles on the side of the hip that help you step laterally—while cyclists tend to have massive quads and tiny glutes.
Note: Exercises that strengthen the hip flexors also involve contracting (shortening) these muscles. So if tight hip flexors are a problem for you, it might be wise to limit how many direct hip-strengthening exercises you perform. These exercises are more geared toward people who have been told they have weak hip flexors that need strengthening or are looking for targeted exercises to build more power and stamina in the hip flexors.
The more than 20 muscles that make up your hips are responsible for stabilizing your pelvis, moving your legs from side to side, and shortening to draw your knees toward your chest every time you sit down, run, jump or pedal, explains Kelly Moore, a certified yoga instructor and co-founder of Mindfuel Wellness, which brings health and wellness initiatives to companies throughout Chicago.
But how can you tell? It can be tricky. This is a concise, readable guide to symptoms that need better-safe-than-sorry investigation with your doctor. (It’s basically just a plain English version of clinical guidelines for doctors.9) In other words, this article explains the difference between “dangerous” and “just painful” as clearly as possible. Tables, checklists, and examples ahead.
Kidneys — The kidneys are a matched pair. One painful kidney can cause back pain on one side or the other. Kidney pain can feel like back pain, and may occur on only one side. It is usually quite lateral, and just barely low enough to qualify as “low” back pain. However, when kidney stones descend through the ureters, they can cause (terrible) pain in the low back. Kidney stone pain is often so severe and develops so rapidly that it isn’t mistaken for a back pain problem.
The sacroiliac joint typically has little motion. Small movements at the joint help with shock absorption and forward/backward bending. The joint is reinforced by strong ligaments surrounding it, some of which extend across the joint in the back of the pelvis. This network of soft tissues provides support, limits movement at the joint, and assists with absorbing pressure.
If you've been working out hard, playing football or other sports, or moving a lot of heavy furniture and are suddenly suffering with radiating lower back pain, chances are you have pulled muscles or strained muscles in the back. Often, strained muscles will cause a burning sensation in the lower back a feeling of having limited movement ability. Doctors prefer to limit prescribing of opiate pain medications and may give you an alternative medication with less risk of becoming dependent.
How to do it: Rest the barbell across the upper traps on the back of your shoulders, as if it’s sitting on the collar of your shirt. Begin with your feet about shoulder-width apart, toes pointing about 15 degrees out, then widen your stance as needed so you’re comfortable. Make sure your knees are pushed out and your glutes stabilize your position. When you’re ready, push your glutes back as if you’re sitting in a chair. Aim for dipping your butt below your knees, but go down as far as you can without bending forward or losing balance. When you get the bottom of your squat, squeeze your glutes and drive up and through your heels back to start position.
How to do it: Stand up tall while holding onto a sturdy object like a chair, or rest your hands on a wall. Keeping your back straight and core tight, raise one leg up and lift it out to the side and away from your body, keeping your leg straight. Pause for one to two seconds, then return to the start position. That’s one rep. Repeat this movement, alternating sides each time.
You can perform this exercise seated in a chair or on your back on the floor. In a chair, cross your left ankle over your right knee and then lean forward until you feel a stretch in your hip. On the floor, cross your left ankle over your bent right knee and then reach under with both hands and pull your right leg toward your head. Perform each variation on both legs.

Tight hip flexors can result in lower back pain, hip pain and injury.  A lot of strain is put on those muscles during activities that involve sprinting and kicking.  For example, runners are more prone to hip flexor injuries because of the small, repetitive movement during running.   But even if you’re not an athlete, hip flexor injuries can occur during everyday activities (for instance, slipping and falling or running to catch a bus).  When those tight muscles are suddenly stretched beyond what they are accustomed to, you might also experience pain in the upper groin region, typically where the hip meets the pelvis.   
I use this for people that really present in an anterior pelvic tilt, or with people that appear to have too loose of an anterior hip capsule.  In fact, this has completely replaced the common variations of hip flexor stretches in all of our programs at Champion.  This works great for people with low back pain, hip pain, and postural and biomechanical issues related to too much of an anterior pelvic tilt.
Muscle Imbalances – The front of your hips, your hip flexors, are the muscles that will tighten and shorten while you are sitting for hours each day. While you are sitting, the back of your hips, your glutes and your hip extensors, are being overstretched. But just because they are being tightened and stretched respectively, doesn’t benefit either of them. They are also being weakened because of the lack of use of each muscle group.
Located deep in the front of the hip and connecting the leg, pelvis, and abdomen, the hip flexors— surprise, surprise— flex the hip. But despite being some of the most powerful muscles in our bodies (with a clearly important role), it’s easy to neglect our poor hip flexors— often without even knowing it. It turns out just working at a desk all day (guilty!) can really weaken hip flexors since they tend to shorten up while in a seated position. This tightness disrupts good posture and is a common cause of lower back pain. Weakened hip flexors can also increase the risk of foot, ankle, and knee injuries (especially among runners) Hip muscle weakness and overuse injuries in recreational runners. 3. Hug it out. Start the supine hip flexor stretch the same as the glute bridge, but keep the right leg relaxed on the floor. Pull shoulder blades down and back to lift hips. Grab the back thigh of the left leg and pull the knee toward the chest. Keep the right leg straight and push its heel into the floor (to feel it in the butt). Hold for 30-45 seconds and switch legs.
In this study, one patient with sciatica was sent for ten MRIs, which produced 49 distinct “findings,” 16 of them unique, none of which occurred in all ten reports. On average, each radiologist made about a dozen errors, seeing one or two things that weren’t there and missing about ten things that were. Yikes. Hamstring squeeze. Use the machine that works your hamstrings; you will either lie on your stomach or sit with a pad behind your knee. Push against the pad, moving your knee up toward the ceiling or backward (depending on which position you’re in). “In other words, bend your knees,” Pariser says. But to avoid cramps in your hamstring muscles, don’t bend your knee so much that your heels are too close to your buttocks.

Acute back pain is often the result of muscle sprains or strains. Sprains occur when your ligaments are overstretched and sometimes torn. Strains, on the other hand, are caused by stretching — and possible tearing — of your tendons or muscles. Though the immediate reaction is pain in your back, you may also experience dull aches or discomfort in your hip.
Hi John, Thank you for the video and instructions. My question to you is that I’m schedule to have a reconstructive hip repair (Laberal tear) in July for my right hip and (second) and told that I have a tear in the right as well. I’ve been suffering from back pain too and know its because of the hips and my sitting because of work. If I can tolerate the exercise, would your recommend to do them? And if so, should I take it down from your suggested reps? I’ve been doing DDP Yoga for the last week and besides general soreness and some discomfort in my right hip, i’ve been able to make it through a full workout as well as do the core exercises. Your response would be greatly appreciated.
For example, your quadriceps muscles are a group of four that are located at the front of the thigh; one of the group members, the rectus femoris flexes the hip, which brings your lower extremity (thigh, lower leg, and foot) forward, in front of you. On the other hand, your hamstring muscles are located at the back of the thigh. When they contract, they extend the lower extremity, bringing it behind you.
The best thing about this stretch is that you can easily adjust where you feel it simply by changing the position of your foot. First, try bringing your foot inward (effectively externally rotating your hip), and repeat the same sequence of instructions. Then, do the same with turning your foot out (internally rotating the hip). Wherever you feel the biggest stretch is likely the version you should be doing more of.
Mobility and exercise (e.g., walking, running, stretching, etc.) help to more evenly distribute the forces of impact and weight through this ball-and-socket joint. As people age or find themselves living a more sedentary lifestyle from (e.g., sitting a lot at work), the wear and tear of the hip joint is less distributed, taking place in a smaller area within the socket.
The hip flexors are the muscles at the front of your hip. They’re responsible for several essential functions. Since they’re so often overlooked, we often forget to stretch them before exercising or engaging in rigorous activities. Tight hip flexors can also be a product of being sedentary. So, if you don’t lead an active lifestyle, or if you spend most of your day sitting at a desk, you’ll be susceptible to hip flexor tightness.

Like quadriceps, the hamstrings are 2-joint muscles. Unlike the quadriceps, though, the hamstrings reside at the back of your thigh. They attach at the siting bones, which are located on the underside of your pelvis. When the hamstring muscles contract, the effect is a pulling of the back of the pelvis down toward the back of the thigh, or a bringing of the lower extremity back behind you.
Strong ligaments and muscles support the SI joints. There is a very small amount of motion in the joint for normal body flexibility. As we age our bones become arthritic and ligaments stiffen. When the cartilage wears down, the bones may rub together causing pain (Fig. 1). The SI joint is a synovial joint filled with fluid. This type of joint has free nerve endings that can cause chronic pain if the joint degenerates or does not move properly.
Kneel on your mat with your thighs perpendicular to the floor and tops of your feet facing down. Bring your inner knees together. Slide your feet apart so they are slightly wider than your hips and press the tops of your feet evenly into the mat. Slowly sit down between your feet. Use your hands to turn the top of your thighs inward. Then, lean back onto your forearms and slowly lower torso to floor. Hold for at least 30 seconds.
The sciatic nerve is the largest and longest nerve in the body. It arises from the spinal cord as a collection of fibers from the lower vertebrae. This nerve is responsible for the sensation of the skin of the foot and entire lower leg except for a small area on the inner side. It also supplies muscles in the anterior, lateral and posterior compartments of the leg.

Approximately 15 degrees of hip extension is required to walk normally. If hip flexors are tight then in order to walk, compensatory movement needs to take place through the lower back causing back pain and premature disc degeneration. Like other joints, if we fail to take them through their full range on a regular basis we eventually lose mobility.

The most common cause of sciatica is a herniated disk. Disks act like cushions between the vertebrae of your spine. These disks get weaker as you age and become more vulnerable to injury. Sometimes the gel-like center of a disk pushes through its outer lining and presses on the roots of the sciatic nerve. About 1 in 50 people will get a herniated disk at some point in life. Up to a quarter of them will have symptoms that last more than 6 weeks.
How to do it: Grip the barbell so you’re standing straight with the barbell at arm’s length in front of your thighs. Kick your left leg back so it’s just off the floor. Bend over so you’re hingeing at the hip while allowing only a slight bend in the right knee, and lower the barbell to the floor while keeping it close to your body. Pause at the bottom, then reverse the movement back up to the top. That’s one rep. Repeat this movement, alternating sides each time.

Start on your hands and knees with your palms flat on the floor and shoulder-width apart. Your neck should be in line with your back, and your gaze should be down or slightly forward. Brace your core, and raise your left arm and right leg until they’re in line with your body. If that’s too challenging, only raise your leg. Either way, hold for five to 10 seconds, and then return to the starting position. Repeat on the opposite side (right arm and left leg) to complete one rep. Aim for five to seven reps total.
“As a result, they’re at greater risk of injury,” says physical therapist Ioonna Félix, clinical supervisor at the Hospital for Special Surgery’s Sports Rehabilitation and Performance Center in New York City. Many times, those injuries manifest themselves in ways that seem entirely separate from your hips, like IT band issues, low-back pain, or plantar fasciitis.
To complete this stretch, take a knee in front of a wall so that the toes of the leg you have raised are pressing against the wall. Place that same hand against the wall. Reach behind you with your other hand and grab your leg that’s sitting on the floor by the ankle, and bend it back towards your body. Hold this position and lunge forward towards the wall to complete the stretch. Hold this stretch for 10 seconds, then repeat on the other side of your body.
The gluteus medius is a muscle that sits on the outside of the hip area. This muscle is used for standing upright and walking. When injured, these muscles will cause a limp. If these muscles are torn, it can cause severe pain when walking, sitting, or sleeping. Treatment usually involves rest, physical therapy, and sometimes surgery to repair the torn muscle, if no other treatments bring relief from pain.
To complete this stretch, get into the same kneeling position from the half kneeling hip flexor stretch. Whichever leg you have raised, place that hand on your hip. (So, if you’re doing this exercise with your right leg, place your right hand on your right hip, and vice versa.) Next, tighten your glute muscles, and reach around your body with your free hand to grab that foot. Pull that foot upwards towards your upper body
Low back pain commonly is experienced in the back itself. However, due to the complexity of the spinal cord and associated nerves being an intricate part of the low back, pain may and frequently can radiate or travel further down the course of the nerves. This is similar to striking your "funny bone" in the elbow and feeling the sensation in the hand below the elbow.
You can use over-the-counter remedies such as Motrin or Advil (ibuprofen) or Aleve (naproxen) to help with pain and swelling. Tylenol (acetaminophen) works for pain relief, but it doesn't treat inflammation and swelling. If you have heart disease, high blood pressure, kidney disease, or if you've had ulcers or internal bleeding, check with your doctor before taking any of these medications.
