The hip is a very stable ball and socket type joint with an inherently large range of motion. The hip contains some of the largest muscle in the body as well as some of the smallest. Most people lack mobility due to a relatively sedentary lifestyle. Periods of prolonged sitting results in tightness of the hip flexors and hamstrings. Tightness in the muscles and ligaments can created joint forces that result in arthritis, postural problems, bursitis, and mechanical back pain.
The hips are the fulcrum point for all lower-body movements. Weak hips can cause an antalgic gait, leading to knee, foot and ankle problems that can keep you off the field and out of the game. Although building your legs with compound exercises like Squats and Lunges is important, you need to set some time aside to focus on your hip muscles by performing hip-specific strengthening exercises. Incorporate these hip-strengthening exercises into your routine to make sure your hips stay strong and injury free.
There are cases of low back pain that have alarming causes, but it’s rare. Once in a while back pain is a warning sign of cancer, autoimmune disease, infection, or a handful of other scary culprits.7 Over the age of 55, about one in twenty cases turns out to be a fracture, and one in a hundred is more ominous.8 The further you are from 55, the better your odds.
Wow this is going to help me a ton! I was just thinking about how I wanted to work on my hips when I was on a 9 mile heavy pack hike yesterday. Even more so when I was done and one of my hips was/is pretty sore. Hips keep us together! Like for real they connect out lower and upper body lol. Need to make sure they are strong, mobile, and flexible which is all something I never really put any effort into improving. I figured my activities like Mountain Biking, Hiking, Climbing, Skiing, and doing squats/lunges along with other exercises would keep them strong. Then I come to find out I only was able to get through 2 rounds… This will now be apart of my training program 🙂 Thanks for the great video! Acute back pain is often the result of muscle sprains or strains. Sprains occur when your ligaments are overstretched and sometimes torn. Strains, on the other hand, are caused by stretching — and possible tearing — of your tendons or muscles. Though the immediate reaction is pain in your back, you may also experience dull aches or discomfort in your hip.
Why does it tighten down so much when overworking as a spinal stabilizer? Remember that length tension relationship we talked about? Well, if the psoas is tight, it can compress the spine easier, thus providing spinal stability. Plus, it has to work a lot to stabilize the spine. When you don’t have correct functioning of the diaphragm and abdominals, the psoas holds a great deal of tension to do the job. This tightness or tension makes it a very ineffective hip flexor.
How to do it: Rest the barbell across the upper traps on the back of your shoulders, as if it’s sitting on the collar of your shirt. Begin with your feet about shoulder-width apart, toes pointing about 15 degrees out, then widen your stance as needed so you’re comfortable. Make sure your knees are pushed out and your glutes stabilize your position. When you’re ready, push your glutes back as if you’re sitting in a chair. Aim for dipping your butt below your knees, but go down as far as you can without bending forward or losing balance. When you get the bottom of your squat, squeeze your glutes and drive up and through your heels back to start position.
“Red flags” are signs or symptoms that something medically ominous may be going on. Red flags are not reliable, and their presence is not a diagnosis. When you have some red flags, it only indicates a need to look more closely. Sometimes red flags are missing there really is something serious going on… and sometimes they are a false alarm.18 Check off all that apply … hopefully none or few or only the least alarming of them!
The sacroiliac joint connects the lower spine to the pelvis and any injury or strain to this area can cause a lot of back, hip, groin, and sciatic pain. Sacroiliac joint dysfunction (SJD) can cause not only radiating lower back pain but can also severe pain in the hip area including hip pain at night when sleeping and hip pain when sitting. Often the pain resembles that of a hip injury it is so severe. SJD can also cause severe pain in the groin area. Women may also notice pain running along the distribution of the sciatic nerve. Treatment for sacroiliac joint dysfunction includes rest, anti-inflammatory drugs, and sciatic nerve massage.
At the very least, the tension and/or spasm in muscles that cross over the hip and attach onto the pelvis can contribute to imbalance, in terms of how strong and flexible each muscle group is in relation to the others. But muscle imbalance in the hips and the spine may make for pain, limitation and/or posture problems. It can also increase the healing challenge put to you by an existing injury or condition, for example, scoliosis.
Often you’ll perform static stretches seated or lying down, and focus on breathing slowly and deeply to facilitate relaxation — sometimes for several minutes at a time. Static stretches can be very effective at loosening you up, but they also inhibit performance in the stretched muscles for a short time afterward. So they’re best reserved for after a workout, or as an anytime stress reliever — just not right before a workout involving the muscles you’re stretching.
Line up your hips parallel to each other, continually pressing the left hip toward the floor. If this position is too difficult, place a blanket under your bottom. To intensify the stretch, move the right foot away from the left side of your body and drop to the elbows or chest. To make this pose less intense, move the right foot closer to your right leg and stay on the hands instead of folding.
That is, the parts of your body that touch a saddle when riding a horse: groin, buttock, and inner thighs. I experienced rather intense, terrifying awareness of symptoms in this area in the aftermath of my wife’s car accident in early 2010. With a mangled T12 vertebrae, she was at real risk of exactly this problem. Fortunately, she escaped that quite serious problem. Kidneys — The kidneys are a matched pair. One painful kidney can cause back pain on one side or the other. Kidney pain can feel like back pain, and may occur on only one side. It is usually quite lateral, and just barely low enough to qualify as “low” back pain. However, when kidney stones descend through the ureters, they can cause (terrible) pain in the low back. Kidney stone pain is often so severe and develops so rapidly that it isn’t mistaken for a back pain problem.

The hip flexor allows you to raise your legs toward your torso. The muscles of the hip flexor are also responsible for keeping your hips and lower back strong, flexible and properly aligned.
Sacroiliac joint pain can occur when movement in the pelvis is not the same on both sides. Uneven movement may occur when one leg is longer or weaker than the other, or with arthritis in the hip or knee problems. Autoimmune diseases, such as ankylosingspondyloarthropathy, and biomechanical conditions, such as wearing a walking boot following foot/ankle surgery or non-supportive footwear, can lead to degenerative sacroiliitis.
To do this stretch, sit on the floor with your legs about three or four feet apart, depending on how tall you are. Make sure your toes and knees are pointed straight up. Next, take a deep breath, and on the exhale, slowly fold your upper body forward. Rest your hands on your feet, legs, or the floor in front of you and hold this stretch for five deep breaths.
In this study, one patient with sciatica was sent for ten MRIs, which produced 49 distinct “findings,” 16 of them unique, none of which occurred in all ten reports. On average, each radiologist made about a dozen errors, seeing one or two things that weren’t there and missing about ten things that were. Yikes. • Psoriatic arthritis. Psoriatic arthritis is a form of arthritis accompanied by the skin disease psoriasis. The skin disease often precedes the arthritis; in a small percentage the joint disease develops before the skin disease. For about 20 percent of people with psoriatic arthritis, the disease involves the spine. In some cases, bony overgrowth can cause two or more vertebrae to grow together, or fuse, causing stiffness.
Back “spasms” are a largely a myth — there’s no such thing a sustained painful contractions of muscles in otherwise healthy people (see Cramps, Spasms, Tremors & Twitches) — but the kernel of truth in the idea of “spasms” may be the idea of trigger points, which are hypothetical “micro cramps,” tiny patches of painfully contracting muscle. Although this idea is controversial, it is nevertheless one of the most likely explanations for common aches and pains that mostly stick to one area (especially the back) and have no other obvious cause. In the vast majority of patients with low back pain, symptoms can be attributed to nonspecific mechanical factors. However, in a much smaller percentage of patients, the cause of back pain may be something more serious, such as cancer, cauda equina syndrome, spinal infection, spinal compression fractures, spinal stress fractures, ankylosing spondylitis, or aneurysm.
How to do it: With your bar on the floor in front of you, place your feet slightly narrower than hip-width apart. Make sure the bar is as close to your shins as possible, and position your hands on the bar, just outside your shins. With your shins perpendicular to the floor, flex your hamstrings and lift your butt up and back, extending your legs so they’re nearly straight, with only a slight bend in the knee. Slide the bar up and toward the shins. Then, bend your knees slightly and proceed to lift the bar all the way off the floor in a straight line. Extend your hips fully at the top and stand tall with neck relaxed, arms straight. Pinch your shoulder blades together, squeezing your abs and glutes. Keeping the bar close to your body and with a straight back, reverse the movement. That’s one rep.

This pose is similar to seated forward fold, and provides the same benefits. It also stretches your groin. Assume the same starting position as the forward fold, but slide the sole of your left foot against your right inner thigh. Keeping your right foot flexed, lower your left knee as far as you can into a half-butterfly position. Walk your hands down the sides of your right leg as you fold forward as far as you can with your torso. Hold for 8-10 breaths, and then slowly rise back up. Switch legs and repeat.
Holland also suggests doing strength work in different planes of motion to keep all the muscles in and around your hip flexors, especially your glutes, firing correctly.“You can’t have good hip flexion if your glutes are tight or weak,” Nurse says, “so it’s super important that you’re always stretching and strengthening the front of your hip flexor and the back, which are the glute muscles.”
Marvelously progressive, concise, and cogent guidelines for physicians on the treatment of low back pain. These guidelines almost entirely “get it right” in my opinion, and are completely consistent with recommendations I’ve been making for years on PainScience.com. They are particularly to be praised for strongly discouraging physicians from ordering imaging tests only “for patients with low back pain when severe or progressive neurologic deficits are present or when serious underlying conditions are suspected.”
3. Hug it out. Start the supine hip flexor stretch the same as the glute bridge, but keep the right leg relaxed on the floor. Pull shoulder blades down and back to lift hips. Grab the back thigh of the left leg and pull the knee toward the chest. Keep the right leg straight and push its heel into the floor (to feel it in the butt). Hold for 30-45 seconds and switch legs.

Sciatica refers to back pain caused by a problem with the sciatic nerve. This is a large nerve that runs from the lower back down the back of each leg. When something injures or puts pressure on the sciatic nerve, it can cause pain in the lower back that spreads to the hip, buttocks, and leg. Up to 90% of people recover from sciatica without surgery.
Piriformis syndrome is not considered a serious injury—so RICE is in order for pain caused by triggers, like sitting too long. RICE is Rest, Ice, Compression, and Elevation. Also, see a primary care doctor or neurologist (nerve specialist), who can recommend physical therapy exercises and exercises for hip pain and other specific types of pain that will help you feel better. Also, a physican therapist can suggest ways exercises that provide pinched nerve relief, as nerve pain relief is what is primarily needed here.
The hip flexors are the group of muscles that allow you to lift your knees toward your chest and bend forward from the hips. What is collectively referred to as the hip flexors is actually a group of muscles that includes the iliopsoas, the thigh muscles (rectus femoris, Sartorius and tensor fasciae latae), and the inner thigh muscles (adductor longus and brevis, pectineus and gracilis).
If the problem originates in the hip joint itself, common symptoms include groin pain on the affected side, and sometimes down the inner aspect of the thigh in the front of the leg. This pain can move to the knee and sometimes feels like a knee problem instead of a hip problem. Walking worsens the pain and with continued activity, the pain increases. Rest relieves it; however, when hip arthritis becomes severe, you may have pain most of the time. Minimal activity such as slight movements while in bed can worsen the pain. Other conditions such as advanced congenital hip dysplasia or avascular necrosis of the hip can cause these symptoms as well.
Tight hip flexors can also make it harder for your glutes to activate—since they're opposing muscle groups, when one is really tight the other becomes lengthened. When a muscle is more lengthened than it should be, it takes away some of its ability to contract. When your glutes are in this compromised position, it can cause other muscles to do more work than they should, making your workouts less efficient and sometimes, increasing your risk of injury.
